Nature:
People refuse to take prescribed medical drugs for many reasons; laziness, failure to understand the consequences of not taking the drugs and belief that the consequences of recovery from the illness are worse than the consequences of the illness.
Incidence:
Russian prisoners refuse to take drugs for tuberculosis because they fear that if they recover, they would be sent to a labour camp, which would be even worse than the squalid prisons they live in.
