Nature

Doctors must be licensed to practice, and hospitals accredited to offer health services. Doctors must pass examinations and hospitals must undergo inspections before being licensed to operate.

Incidence

An American hospital lost its accreditation after amputating a patient's wrong leg, wrongly removing another patient from a ventilator, and undergoing a surprise inspection. The loss of its licence means that the hospital can no longer receive USA government funding, which accounted for about one third of its revenue.
